All participants must be 18 years or older to be eligible to win or claim the prize but a parent can enter on behalf of a child. One Entry per patient. There is no household limit. Offer is valid for New or Existing Patients only. Mention this promotion when making your appointment. Not valid with any other discounts or promotions. Void where prohibited by law. Other restrictions may apply. Prize is valued at $5,200. All prizes are awarded “as is.” Prizes are non-transferable and cannot be exchanged for other trips, prizes, or cash.
