Contest will run from May 27th – June 30th & one winner from each office will be announced on July 15th.

Contest Rules for New Patients:

To be eligible to win, contestants must schedule their new patient consultation with the “New Patients Enter Here” form above by June 30th and treatment must be started by July 15th. One entry per patient. Must be 18 years or older to win. Employees and immediate family members of NCOSO are not eligible to win. Winners will be chosen at random and contacted via phone or email to claim their prize. Prizes must be claimed by July 19th at 6 pm.

To be eligible to win, fill out the contact form with your home office between May 27th and June 30th & be entered to win an NCOSO Yeti Cooler Prize Package. One entry per patient. Must be 18 years or older to win. Employees and immediate family members of NCOSO are not eligible to win. Winners will be chosen at random and contacted via phone or email to claim their prize. Prizes must be claimed by July 19th at 6 pm.
